It is very important that before you issue a survey, you know how you're going to analyze the data and use the results to make decisions. Otherwise, you'll be saying, "I wish we would have..."

Here is a site that has some info on locker rates, even though it's from 2000. It looks like they are in the $75-85/year, not counting key deposit. Some other good info on city bike parking ordinances as well.

If you have additional ideas on what we should try to learn with the survey or how we should construct the survey, please reply-all with them.

New York City also has a nice survey at http://www.surveymonkey.com/s.aspx?sm=1b9Lp_2fkA5UvV3B_2f1T8Dl4w_3d_3d It has questions about bike parking but also broader questions whose answers could be used to make decisions about other matters than might make Morgantown more bicycle friendly. I think some of the answer options are too broad or too constrained to be useful for making required decisions. e.g. One question asks which part of town you'd like to see more bike parking (parts of NYC are pretty big!) another question asks, How much would you be willing to pay per month and the first choice is 0-$25. (Some more subdivision may be useful.) Nevertheless, the survey questions can help us decide what to ask. I'll reiterate though that before we start asking questions, we must know how we're going to use the answers.
