Boulder has a nice bike parking survey at http://www.surveymonkey.com/s.aspx?sm=HEGcw8m8r6CKeyP7qdy58w_3d_3d  You can step through it without having to answer anything.  I think it contains the questions we want to ask.  

It is very important that before you issue a survey, you know how you're going to analyze the data and use the results to make decisions.  Otherwise, you'll be saying, "I wish we would have..."

Here is a site that has some info on locker rates, even though it's from 2000.  It looks like they are in the $75-85/year, not counting key deposit.  Some other good info on city bike parking ordinances as well.
